You can become a registered member of St. Andrew´s Catholic Church by stopping by the Parish Office to fill out a registration form during the hours of 9:00 am - 4:00 pm, Monday through Friday. The following information is needed about yourself and any other household members:

• Your contact information: Address, phone number
  
• Names, ages and genders
  
• Occupation or School
  
• Religion: Baptised Catholic, Baptised non-Catholic, Unbaptised, or other.
  
• Sacraments Received: Baptism, Penance, First Communion, Confirmation...
  
• Marital Status: single, married by priest, married by non-priest, separated, divorced, widowed
  
• Any special needs: deafness, blindness, homebound, mentally challenged, little or no English, or other.
  
• Preferred Language
